Patient Registration Form
At your first appointment you will need to fill out a New Patient Registration Form with your details. Download by clicking the following button:
Please bring the following items to your consultation:
-
We are a Specialist Practice and patients need a written referral from their General Practitioner, family physician or other doctor, to be eligible for a rebate from Medicare Australia. A GP written referral is valid for 12 months, whereas a referral from another specialist is only valid for 3 months.
-
Your Medicare Card and Private Health Fund details (where applicable).
-
Email in previous correspondence, reports, and investigation results (such as pathology and imaging tests) before your appointment. Bring in your child’s Personal Health Record (The Blue Book) to the appointment.
Cancellations
Please telephone the office during business hours or leave a message. Please give at least 48 hours notice for a cancellation, so that your appointment time can be allocated to another patient on the waiting list.
We make every effort to run on time, however emergencies, providing all parents with the time they need, and other issues can cause delays beyond our control. We do apologise if you are kept waiting.
Consultation Fees
Payment is required at the time of the consultation. We accept Visa, MasterCard, Eftpos, or cash. We do not accept American Express or Diners Club.

Fees charged in this practice are based around our professional associations’ recommended fees and the costs to cover the sustainability of a private practice. We are not a bulk billing practice. There will be a portion of your consultation fee which will not be covered by Medicare or your private health fund (the "Gap"). It is important that you understand this gap prior to the consultation. Our reception staff are happy to answer any questions you may have in regards to the fees.

The Medicare Safety Net provides a higher Medicare benefit for out of hospital costs. For more information see Medicare Safety Net on the Australian Department of Human Services website.

Admission to hospital as a Private Patient allows you to choose the specialist who will look after your child, otherwise you will be allocated the doctor on-call at the time.
